A Message from Athletic Director Christopher Schiumo
Dear Sweet Home Families,
We are very excited to get our Fall 2025 sports season started! Sweet Home Central School District Athletic Department will continue our use of FinalForms, an online forms and data management service to sign up for all athletics. FinalForms allows you to complete and sign athletic participation forms for your child. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your children, saving you time. Those of you who played a sport in the 2024-2025 school year, your data will roll over to the Fall season.
It is required for all parents and guardians to sign up athletes through FinalForms. Please complete this before August 18, 2025 for Fall High School and Modified Sports.

and follow the prompts to create your account and sign your forms. Once this is complete, your child will receive an email to login to their account and sign select forms. Students may not create their own account. They will receive an automated email once the parent account has been set up.
Our Health Office and our District Medical Director will be offering free physicals for any student who needs a physical for athletics in the 2025-26 school year. This will be done at the High School on August 14th, 2025. Girls will be at 5:30 p.m. and boys will be at 6:00 p.m. All students can enter through Door 4 at the HS and report to the Health Office. I encourage any athlete who does not have a current physical to attend this event. This is open for current 7th-12th grade students.

Transfer Students Interested in Sports
A student who transfers to Sweet Home CSD in grades 7-12 must complete the NYSPHSAA Transfer Request Form. Please return completed transfer forms to the Sweet Home Athletic Office located in Sweet Home High School.
